Your tears are secreted by the lacrimal and accessory lacrimal glands. The eye lids act as windshield wipers which spread the tears evenly across the cornea polishing the optical surface. The tears are a combination of aqueous, lipid and meibomian fluids. The tears are an important source of microbial substances, such as lysozymes, minerals and enzymes. These substances are important for fighting microbes which are found on the surface of the eye. Natural tears also provide nourishment to the top layers of the cornea.
Dry eyes might be caused by one or more of the following reasons
1. The productions of natural tears may be inadequate.
2. There may be increased evaporation or increased drainage.
3. Environmental considerations such as living in a dry climate or having certain allergies
· Aging
· Menopause
· Allergy Response
· Environmental Stresses
· Contact Lens Wear
· Wind
· Air Pollution
· Ocular Surgery such as LASIK or Corneal Transplant
· Medications/ Topical Medications
· Preservatives in Artificial Tears
Systemic
· Antihypertensives
· Antiandrogens
· Anticholinergics
· Antidepressants
· Cardiac Antiarrhythmic drugs
· Parkinson's disease agents
· Antihistamines
When the eyes are deficient in tear volume, either tears must be added or their drainage from the eye decreased. Artificial tears in the form of drops or ointment may be instilled. The production of tears may also be increased with a prolonged regime of anti-inflammatory drops.
Another way to increase tear fluid levels would be to decrease the drainage of tears using punctual occluders. Similar to a stopper in a sink, this is a painless procedure done in the office prior to or even the day of your LASIK procedure. You will not feel them in your eyes.
Punctal Occluders
| Type | Temporary | Intermediate | Permanent |
| Material | Collagen | Synthetic Polymer | Silicone or Acrylic |
| Duration | 2 - 6 weeks | up to 3 months | Lasts for years |
| Usage | Diagnostic | LASIK / Therapeutic | Therapeutic |
| Long Term | Dissolves | Slowly Dissolves | Inert |
There are various types of occluders. They come in different materials (refer to table). Depending on the patient's needs, the right type will be determined and in some cases insurance will cover the cost.
